Add Checkbox Field

Viewer need to check 'one' option from the list.  See Form Example.

Begin on Quick Panel, select the Palette tab. Then click on Form within the Group tabs. Click, hold and drag Checkbox Field icon on to your web page. The Checkbox Field Properties dialog box will display. Let's review each section: Label Type in name of field Required Place a checkmark in box if the data must be collected in the form Billing Options Optional -- Section used for credit card payments. Adds an Amount Check this box to use the amount provided to accept credit card payment.  Amount to Add Enter the amount for this field. Since this field is static, the user will not be able to change it and it will always display this value. .  Complete or Cancel Select OK to save the field properties Select Cancel to abort operation.

To add more field types, just drag the appropriate Field Type onto your form. Each Field Type will be added to the bottom.  Need to move a field up or down within your form, see Move Field within Form for more information.
